The Weld RE9 School District has a food allergy and anaphylaxis policy which requires us to provide parents with the standard allergy and anaphylaxis form (found to the right on this page). If your child needs an epinephrine delivery device at school to treat severe allergies, please have your child's health care provider complete this form.
